Note: Crowsnest Physio does not receive any funding from Alberta Health Services and does not direct bill insurance companies at this time. You are required to pay in full the $115.00 for initial assessments and $95.00 for follow-up appointments. If you are currently dealing with an insurance company due to a work-related injury or motor vehicle accident, you are required to pay directly and seek reimbursement from your insurance company unless prior arrangements have been made in writing with our billing and accounts department. Please email reception@ferniephysio.com with your full legal name, personal health care number, claim number and which insurance company (or Alberta WCB) you have your claim with, to be considered for direct billing.
